To reserve your space in the 2005 Oregon Ceramic Association Show Book, fill in the top section, check the appropriate space below, date, sign and return this form.
The deadline is February 1st, 2005. If Payment is not received by the February 1st deadline, your advertisement will not be published and any deposit will be forfeit. (An exception may be made, if arrangements are made prior to the deadline with the Show Book Chairman for a later payment date.)
Before submitting your contract, be sure to date and sign the bottom of the page. Please, do not forget to include your check according to the size of the ad you want. A deposit of 50% must be included with this order form.

If you do not have the means to create an electronic file, we can work with you
